Creating a Patient Face Sheet File

SigmaMD enables clinicians to craft a comprehensive patient face sheet using the referral file creation tool. Let’s explore how you can utilize the referral creation process to generate a patient face sheet file.

 

Adapting the Referral Tool for Face Sheet Creation

1. Initiating the Process:

To begin, click on the "Referral" button in the Overview section of SigmaMD's interface. This action opens a modal that guides you through a two-step process to compile patient information.

2. First Step: Customizing Information:

In this step, select the pieces of patient information that are crucial for a face sheet. Customize the content by selecting from:
- Insurance information
- Patient conditions
- Medications
- Vital measurements
- Vaccinations
- Procedures

The default selections aim to save time, but you have the flexibility to adjust these options as needed.

3. Skipping the Cover Page:

Since the goal is to create a face sheet and not a referral, omit adding a cover page to your file. The cover page, typically used for referrals, includes details like the recipient name, referral reason, and a note, which may not be necessary for a face sheet.

4. Second Step: Including Additional Data:

You may choose to include additional relevant information such as:
- Encounter notes
- Insurance card picture
- Driver's license picture
- Lab results

Select these based on their relevance to the face sheet you are creating.

5. Generating the Face Sheet File:

After customizing the information, select "Generate file". The platform will then create a PDF file, ready for download and use.
