Troubleshooting Patient Web Enrollment
Error Codes
Enrollment could not be completed. Contact the office.
If a patient receives the error message “Enrollment could not be completed. Contact the office.” while attempting to enroll through your web enrollment link, it typically means that their chart already exists in SigmaMD. This is common when the patient has previously been seen for a visit prior to enrolling in membership.
What to Do
In this case, the patient cannot use the self-enrollment web link. Instead, follow these steps to manually attach a membership:

By manually assigning a membership, you can ensure the patient or family is properly enrolled without needing to create a duplicate record.

Enrollment could not be started
If the patients are encountering the "Enrollment could not be started" error when using the enrollment link, it's typically because the patient's chart already exists in the system. Here's how to resolve this issue.
Error Message Example:
When trying to enroll a patient, you might see the error: Enrollment could not be started. Please contact the office for assistance.
This occurs when the system detects that the patient is already in SigmaMD based on their email address.
Solution:
- Check if the Patient Exists in the SigmaMD:
- The error happens if the patient already exists in the SigmaMD. In this case, you can’t enroll them through the enrollment link.
- Instead, attach the membership through the SigmaMD.
- Create the Patient Manually (If Needed):
- If the patient is not in the SigmaMD, create their chart manually and then proceed with the enrollment.
