Enrollment Error When Using the Web Enrollment Link

Common Error Messages

  • “Enrollment could not be completed. Contact the office.”
  • “Enrollment could not be started. Please contact the office for assistance.”

What These Errors Mean

These errors typically indicate that the patient already has an existing chart in SigmaMD. This often occurs when:

  • The patient was seen for a visit and has a chart, or
  • The system detects an existing chart based on the patient’s email address.

To prevent duplicate records, patients with existing charts cannot complete enrollment using the web enrollment link.

What to Do

Attach the Membership Manually

If the patient already exists in SigmaMD, attach the membership directly from the EMR.

If the Patient Does Not Exist

If the patient does not yet have a chart in SigmaMD, create the patient manually and then proceed with enrollment.
