How to Set a Medication as Completed, Discontinued, or Mistake in the Patient Chart
SigmaMD allows you to manage a patient's active medications directly from their chart, including marking medications as completed, discontinued, or entered by mistake. Here's how to do it:
Mark a Medication as Completed, Discontinued, or Mistake
- Navigate to the Medication Screen
Open the patient chart and go to the Medications section. - Hover Over the Medication Name
Move your cursor over the medication you'd like to update. - Click Remove
Select the Remove button, and you’ll be presented with three options:- Completed
- Discontinued
- Mistake
- Provide a Reason (Optional)
For Completed and Discontinued, you have the option to provide a reason. If filled out, this reason will be visible under Manage Details for that medication. - Confirm Your Selection
After making your choice, click Confirm to save the change.
Reactivating a Medication
If a medication was marked as Completed or Discontinued, you can set it to Active again:
- Hover over the medication name and click Set to Active.
Note: Medications marked as Mistake cannot be reactivated.
Syncing with DoseSpot
Changes made in the SigmaMD patient chart will sync to DoseSpot when the Rx modal is opened. Likewise, changes made in DoseSpot will sync back to SigmaMD when the modal is closed.
To Cancel a Prescription
If you need to cancel a prescription (send a cancel message to the pharmacy):
- This must be done from within the DoseSpot modal.
- Canceling is not supported from the Medications screen in the SigmaMD chart.
