How to Manage Diagnoses on Appointments

You can add diagnoses directly to appointments, bringing clinical context into scheduling and keeping patient records aligned across visits.

Add a Diagnosis

  1. Go to the Calendar and click “+ New”.
  2. Select a patient and fill in the appointment as usual, including the appointment type, visit type, clinician, date, and time.
  3. In the Diagnosis field, search and select one or more codes. The list shows codes you’ve starred as favorites first, then codes from the patient’s active problems with the On problem list icon, then all other matching ICD-10 codes.
  4. Click Create to save the appointment.

You can also update current or past appointments and add diagnoses by clicking “Edit event”.

Add Multiple Diagnoses

  • You can add more than one diagnosis per appointment.
  • Diagnoses will appear as labels within the appointment.

Set Default Diagnoses For Appointment Types

You can configure appointment types with predefined diagnoses. These will automatically populate when the appointment type is selected.

  1. Go to Practice Settings → Scheduling → Appointment types.
  2. Select an appointment type to edit or create a new one.
  3. In Coding, add the default diagnoses in ICD-10 and click Save.

Require a Diagnosis To Mark Appointments As Attended

If your practice requires a diagnosis before marking visits as attended, turn on Require ICD-10 in the appointment type’s Coding section.

Keep in mind:

  • You can create an appointment without selecting a diagnosis, even with this option enabled.
  • A default diagnosis is not required to enable this setting
  • A diagnosis is only required when the appointment status changes to completed, checked-in or checked-out. Without one, the status shows Diagnosis is required to mark as attended. Add a diagnosis and click Save.
