Before going live on SigmaMD, we recommend sending your patients an email to let them know about the transition and what to expect. You can modify and send the template below as a broadcast message from your current EMR or CRM system. Be sure to replace [Patient First Name] and [Practice Name] with your mail merge fields before sending.

As part of this transition, we're inviting you to register to the SigmaMD patient portal, which will make it easier for you to:

• View prescriptions
• Self-schedule appointments
• Attend televisits
• Access your membership invoices and payment methods
• And much more

Look for an invitation in the near future. Please do not attempt to register to the patient portal until you receive a formal invitation via email with a unique invite code. Please do not reach out to SigmaMD requesting the invite code — they are not able to provide it.
