Locking Forms

Locking a form stops a patient from opening or submitting it, while preserving anything they've already entered. Use it when you need to close a form to further edits, for example, when a patient sent the wrong information, an appointment was cancelled, or you no longer need a response.

When You Can Lock a Form

You can lock a form while its status is Not started or In progress. Any information the patient has already entered is saved, and the patient can no longer access the form once it's locked.

Locking a Form

  1. Open the form and select Lock form from the actions menu.
  2. On the confirmation modal, click Confirm.

The form's status changes to Locked, and the form records who locked it and when. This is also captured in the Activity log.

If a patient tries to open a locked form, they see a Form Locked screen directing them to contact their care team.

A locked form supports the same actions as a submitted form. You can still view responses and attachments, add information to the chart from the Entry Review tab, review the activity log, and mark the form as reviewed.

Unlocking a Form

If your practice locked the form, you can reopen it:

  1. Open the form and select Unlock form.
  2. Click Confirm.

The patient regains access and can continue where they left off.

Note: Once a locked form has been marked as reviewed, it can no longer be unlocked.
