In the realm of patient care, efficient communication between healthcare providers is crucial. SigmaMD recognizes this and offers a streamlined process for creating patient referral files in a PDF format. While our platform is gearing up to support direct faxing capabilities next year (more details here: Interim Fax Solution), we currently provide a robust solution to generate a comprehensive PDF referral file.
Step-by-Step Guide to Creating a Referral PDF
-
Initiate the Process: Begin by clicking on the "Referral" button located on the top of the overview section, in the center of the Patient Chart. This action opens a modal for file creation.
-
Customize Your Referral: In the first step, personalize the content of the referral. Here, you can choose what information to include, such as:
- Insurance information
- Patient conditions
- Medications
- Vital measurements
- Vaccinations
- Procedures
While the main options are preselected based on availability to expedite the process, they can be fully customized per your necessity.
-
Add Cover and Details: A cover page is added by default to the referral PDF, but it can be removed if preferred. This page can feature:
- A title
- Recipient's name
- Referral reason
- A personalized note
- ICD-10 codes
- Your signature
-
Select Additional Information: In the second step, you have the option to include:
- Encounter notes
- Insurance card picture
- Driver's license picture
- Lab results
These are added if available and relevant to the referral.
-
Generate and Download: After customizing your referral, click "Generate file". The PDF will be created and available for download, ready to be sent to the receiving clinician.
If a new tab with your file doesn't open, please check if your browser is blocking pop-ups from sigmamd.com.
Your PDF will look like this:

This functionality ensures that you can quickly and effectively create detailed referral documents, encapsulating all necessary patient information in a professional and readable format.
