SigmaMD allows caregivers to be assigned to patients who require assistance in managing their health records and communications. This feature is essential for ensuring proper care coordination for minors and individuals without direct access to their health information. Below are the guidelines for assigning and managing caregivers within SigmaMD.
When to Assign a Caregiver
A caregiver should be assigned in the following scenarios:

For Patients Under 16: SigmaMD requires a caregiver to any patient under the age of 16.

For Patients Without Email or Mobile Number: If a patient lacks a personal email and mobile number, a caregiver with these details must be linked to their chart.

Caregivers Cannot Be Added Retroactively: A patient who is an independent patient (not a care recipient) and does not currently have a caregiver cannot be assigned a caregiver after the fact. However, if a patient is already a care recipient, additional caregivers can be assigned by following the steps below.
How to Add a Caregiver

Navigate to the patient’s chart in SigmaMD.

Locate the Caregiver section.

Click Add Caregiver and select an existing patient from the system.

The caregiver must be an existing patient in SigmaMD.

The caregiver cannot be a patient who is a care recipient.

Click Save.
What Happens Next?

The new caregiver will receive a notification and will be able to access the patient’s chart in their SigmaMD Patient App.

If they already have the app, the patient will automatically appear in their care recipients list.

The caregiver can now receive notifications, participate in telehealth visits, and manage the patient’s health data.
How to Remove a Caregiver
A caregiver can be removed in the following scenarios:

The patient turns 16, at which point they can manage their own chart.

The patient has obtained a personal email and phone number, making a caregiver unnecessary.
Steps to Remove a Caregiver:

Navigate to the patient’s chart.

Locate the Caregiver section.
-
Click Remove Caregivers.

Confirm the removal.
Important Notes:

Removing a caregiver does not impact the patient’s health records.
-
The patient will still have full access to their existing records in SigmaMD.
SigmaMD’s caregiver management feature ensures that patients receive ongoing support when needed. As a patient grows older or gains access to their own communication channels, their caregivers can be adjusted accordingly, providing a smooth transition to self-management.
