SigmaMD’s platform encompasses a feature designed to ensure that all patients, regardless of age or contact information availability, receive proper care coordination. The concept of 'Caregivers' within SigmaMD plays a pivotal role in managing the health data of patients who may not be able to do so themselves.

When to Assign a Caregiver

A caregiver must be assigned in two specific scenarios:

For Patients Under 16: SigmaMD mandates that any patient under the age of 16 has a designated caregiver linked to their chart.

For Patients Without Email and Phone: If a patient lacks a personal email address and phone number, a caregiver with these contact details must be associated with the patient's chart.

Who Can Be a Caregiver?

The caregiver needs to be another patient within SigmaMD who has an email and mobile phone number. This ensures that there is a reliable channel of communication for appointment notifications and health updates.

Caregiver Notifications and Access

Upon assignment as a caregiver, the individual will receive a notification prompting them to download the SigmaMD Patient App. If they already possess the app, the new patient will automatically be added to their list of care recipients. Caregivers are equipped to receive all pertinent notifications, engage in chat conversations, access the patient's health data, and participate in telehealth sessions concerning the patient under their care.

Transitioning From Caregiver Management

Once a patient reaches the age of 16 or upon acquisition of their own email and phone number, the caregivers can be removed from the patient's chart. This transition allows the now-independent patient to take charge of their health data within SigmaMD. Importantly, this change does not affect the patient's historical health records, which remain intact and accessible on the platform.

In SigmaMD, caregivers are entrusted with a crucial role, bridging the gap for patients who are either too young or otherwise unable to manage their health data and communications.
