The patient intake process in the SigmaMD App is designed to be simple and efficient. Follow the steps below to complete the intake form.
Step 1: Start the Intake Process in the SigmaMD Patient App
- see a card prompting them to complete their intake form.
- Tap the button to proceed.
Step 2: Enter Personal Information
- The patient will first be asked to provide basic personal details and their address.
Step 3: Proceed to the Next Step
- Once all required fields are completed, scroll to the bottom and tap Next.
Step 4: Upload Photo ID
- The patient will need to upload a photo of their ID.
- Tap Upload, then choose to either:
- Take a new photo.
- Select an existing image.
- Only one image is required, even though the app provides options for front and back.
Step 5: Complete the Lifestyle Questionnaire
- The patient will answer a series of questions about their lifestyle.
- Some answers may trigger additional required fields, which will be clearly labeled.
Step 6: Enter Health Records
- The patient will be presented with different health record (HR) sections.
- None of these fields are mandatory.
- To add a health record:
- Start typing in the field.
- The Add button will become enabled.
- Tapping Add will list the record as a badge below the text field.
Step 7: Enter Family History
- The next section allows the patient to add family history.
- Tap Add Relative to enter details.
- Enter conditions for each relative just like the HR fields (type and tap Add).
Step 8: Women's Health
- This optional section allows female patients to add women's health history.
- Enter obstetric (OB) and gynecological (GYN) information respectively.
Step 9: Complete the Intake Process
- Once satisfied, tap Done to submit the intake form.
- All submitted information will be available for the clinician to access in the SigmaMD EMR.
Important Note: Data Review & Integration
While most patient intake data is automatically incorporated into the patient's record, some information—such as problems, medications, procedures, allergies, vaccines—requires review by the care provider before being added to the patient chart. This ensures accuracy and clinical relevance.
