Adding Health Records to a Clinical Note

SigmaMD allows clinicians to efficiently integrate patient health records directly into clinical notes. Follow these steps to add new or existing health records to your note.

Adding New Health Records to a Clinical Note

If the health record has not yet been added to the patient chart, you must create it first.

• Click Add Record to begin adding a new record.
• Ensure a clinical note is open in draft mode—if no draft exists, the option to include the record in the note will be disabled.
• In the pop-up window, check the "Add to Encounter Note" box (this is not selected by default).
• Once saved, the health record will be added to both the patient chart and the clinical note.

Adding Existing Health Records to a Clinical Note

If the health record is already present in the patient chart, adding it to the clinical note is even easier:

• Locate the relevant health record.
• Click the Add to Note icon.
• The selected record will be inserted at the last cursor position in your clinical note.
