This guide walks parents and caregivers through the process of enrolling children into a Pediatric Membership. The caregiver will have access to the child’s chart and will also be the payer for all registered members under the membership.
Step 1: Access the Enrollment Link
Navigate to the enrollment link provided by the practice or access it from the practice's website.
Step 2: Enter Caregiver Information First
- On Step 1 of the enrollment form, enter your own information first as the caregiver or parent.
- Toggle ON the option: "I am enrolling others, but not myself."
- This ensures that a patient chart is created for the caregiver/parent, but they are not included in the membership charge calculation.
Important: Do NOT enter the child’s information first.
Step 3: Add Each Child
- After entering your caregiver/parent details, proceed to add each child one by one.
- The system will automatically assign each child to the appropriate membership seat based on the age rules set by the practice.
- For each child, toggle ON the option: "[Parent’s name] is the assigned caregiver."
- The system automatically enables this toggle for any member under 16 years old.
Step 4: Continue Registration
- Select a Care Provider
- Choose a care provider from the available options.
- Select Membership Start Date & Complete Payment
- Choose the membership start date.
- Enter a payment method.
- Accept all practice terms and agreements.
Step 5: Registration Completion & Access to SigmaMD
- After successful registration, the caregiver/parent and all children will have their patient charts created.
- The caregiver/parent will receive an email with a registration code to sign up for the SigmaMD Patient Mobile App.
- Through the app, the caregiver/parent can access all linked child patient records.
- Children are registered to the membership, but the caregiver/parent is NOT—they are only set as the payer for the membership.
