Release Notes - December 19, 2025
This week’s SigmaMD update focuses on improving patient intake, enhancing the Patient App experience, and expanding Scribe’s language support.
Web-Based Intake Forms
You can now send web-based Intake Forms to patients, allowing them to complete forms without downloading the SigmaMD Patient App.
Intake Forms support questions that can be reviewed from the patient's chart, and information can be added to the chart in a few clicks.
These chart-connected questions aren’t limited to intake forms, they can be added to any form, helping you standardize data collection across workflows.
Patient App Enhancements
Several updates make the Patient App easier to use and more informative:
- Patients can upload a profile picture.
- Practice name and logo are now on the home screen. A link leads the patient to see more information.
- A new Forms section is available under the Care tab.
- A chat shortcut with an active chat counter helps patients quickly return to ongoing conversations.
Multi-Language Support in Scribe
Scribe can now listen to Spanish and many other languages while outputting the note in English. This helps ensure documentation stays aligned with real-world care delivery. Please take a look at the complete list of supported languages.
Alternate Phone Numbers on Patient Charts
Patient charts now support an alternate phone number, including landline and VOIP options. This gives your team more flexibility when documenting and contacting patients.
