How to Use Scribe in the Patient Chart
SigmaMD Scribe allows clinicians to capture voice-based clinical notes in real-time. This guide walks you through how to use Scribe effectively during a visit.
Prerequisites
Before using Scribe, make sure your practice has at least one Scribe-enabled note template.
- You can use the default template provided by SigmaMD, which follows the SOAP format.
- To create or customize your own templates, follow this guide.
How to Start Scribe
If your account is enabled for Scribe, you can initiate it from the patient chart:
- Open a patient chart.
- Click the Start Scribe button at the bottom-left corner (next to the Televisit button).

- Select the note template you wish to use and click Start Recording.

Scribe will begin with a 3-second countdown (3-2-1) and then start capturing audio.
Ensure your microphone slider is at least 3/4th of the way to the right (75%) or higher in system settings for optimal accuracy.
During the Visit
- Pause/Resume Recording: You can pause and resume the recording at any time.
- Timer: A timer displays the recording's current length.
- You can navigate away from the chart while Scribe is recording—your session will not be interrupted.
Generating the Note
- When the visit is complete, click Generate.

- Scribe will transcribe the voice recording and generate a clinical note.
- This can take anywhere from a few seconds to a few minutes depending on visit length.
- The note will follow the structure of the selected template.
Reviewing and Editing
- Click the Sources button to view the full visit transcript.

- You can use the Regenerate note if:
- The note didn’t fully generate.
- You updated the underlying template and want those changes reflected.
Be sure to review the note for accuracy and completeness before signing.
Browser Limitations
Please note that when using the browser Safari, your microphone can be used by only one tab at a time. This means that if you are recording a Scribe note in SigmaMD and start using the microphone in a new tab as well, Safari will stop recording in SigmaMD — and vice versa.
